Anything new in sense?

If this is a newer version of the leak that everyone else got a few weeks ago then no. It's still Sense 5.0 with only minor adjustments. Hopefully this leak will run better then the prior one.

Does applying this take you off the official update path? If i dont care about root/s-off and just applied it via recovery, would I be able to receive new OTA's?
From what I know, this is what will be in the OTA. In any case you can always grab one of the older RUU files and flash it to get back to receiving OTA updates.

How do I flash this on my one? I am not rooted and have used RUU before but this file does not containe the setup.exe file..
Boot into bootloader and use fastboot

fastboot oem rebootRUU

*wait for the HTC logo and your computer to recognize the phone*

fastboot flash zip *zip name/location*

You will need to be S-OFF to do that.

The flashable radio zip isn't sticking at all. Might need to check it
